Most (if not all) already mentioned, but anyway:
  • Aquarius by Haken. Thumbs Up
  • Metropolis pt.2 by Dream Theater.
  • Still Life by Opeth.
  • Human Equation by Ayreon.
  • Seventh Son(...) by Iron Maiden

I also didn't know that Pain of Salvation's Remedy Lane and  Perfect Element pt.1 were concept albums Stern Smile but if that is the case, they definitely deserve mention too.

I think it is once again appropriate to call upon The Dwelling.  After all, it has to be a concept album if it's only one song right?
