Are there any extreme metal bands that incorporate country, western, or delta blues? I particularly want black and doom bands, but other subs are welcome.

For reference, Swans' The Seer is an album that incorporates country in a satisfying, atmospheric, and dark way. That's kind of along the lines of what I'm looking for, just a little more metal maybe.

And I know Earth went doom country later on. No need to mention them.

I still haven't gotten around to hearing this album, but I've heard people mention that are a lot of bluegrass influences here. It's primarily black metal. Thumbs Up

Not really what you're looking for, but definitely country, if not very extreme...

Not heard of any black/death + country hybrids but 3/4 of Pantera made Rebel Meets Rebel with the country musician David Allan Coe: http://www.metalmusicarchives.com/artist/rebel-meets-rebel

Across Tundras might appeal to you. Enjoy them myself. Mix doom / sludge with prog and country / Americana.

Try Bandcamp where most releases are name your price / free - http://acrosstundras.bandcamp.com/

Here's a couple. Anthrax and GBH. Only single songs I know, but it's a bit of an odd combo.

Raging Slab. Has always been described as Skynyrd meets Metallica, but I've never been able to hear the Metallica part.

Thus far, Panopticon and Across Tundras have been closest to my interests. I would think that there's some Southern doom band with some gothic country influences or something. Maybe I'll have to make it myself. Lamp
