The colour and style looks like several well known black metal covers but the shape of the hand doesn't match any of them that I can think of right now.

A first hint is that the band kept the same vocalist and guitarist throughout their whole career, and this album was the first appearance of the bassist that would remain in the band.

No, that's the band's doing. Though I won't pretend that I didn't purposely choose this to see if it caught anyone out.
If you look the colours are also ever so slightly different.

Well I know it and did all along but I also was curious to see if anyone walked into the trap.
It's actually right there on the page for Pagan Black Metal if you scroll down to recent reviews. I remembered this one because when it came out I thought about what kind of gall the band must have to use the same art (albeit mirrored) as one of BM's most famous albums. Got to find a couple of images now to try you all on.
